The messaging app introduced emoji reactions back in May, allowing its users to choose between only six emojis: crying face, heart, thumbs up, tears of joy, joined hands, and mouth open.

To use any emoji reaction, you should long-press on a message you want to react to, tap on the plus button, and pick an emoji you like from the full list. The emoji you picked will be displayed under the message.
Telegram has also recently unveiled its Premium subscription, which allows users to use new exclusive emoji, such as 👌😍❤️🔥💯🤡🌭🐳, to react to messages.
